The following policies and procedures help us ensure that each of our patients receive exceptional medical care in an efficient and patient friendly atmosphere.
Payment of the patient responsibility portion of your visit is due at the time of service including all applicable co-payments.
For your convenience, we accept cash, checks, Visa, MasterCard, and American Express.
All returned checks are subject to a $25 handling fee.
All prescription refills should be submitted via facsimile by your pharmacy. Please allow 24 hours processing time for all prescription requests. Our fax number is:

For the comfort of our patients and to ensure smooth practice operation, please refrain from cellular telephone use during your visit.
Due to space constraints, we ask that all ultrasound visitors be limited to two (2) per patient.
In an effort to maintain accurate records, we will ask that you complete a demographic sheet one per year.
To ensure timely appointments, we may ask you to reschedule your appointment if you arrive more than twenty (20) minutes after your schedule appointment time or if you are unable to present your insurance card upon check-in.
Unfortunately, we do not validate parking.
